THE INFLUENCE OF DIGITAL HEALTH COMPETENCE ON HEALTHCARE SERVICE PERFORMANCE AT KING ABDULLAH MEDICAL CITY, 2026: A QUANTITATIVE APPROACH USING STRUCTURAL EQUATION MODELING (SEM-PLS)

Abstract: Digital transformation has fundamentally reshaped healthcare service delivery worldwide, particularly in tertiary hospitals that rely heavily on integrated digital systems such as Electronic Health Records (EHRs), telemedicine,&#8230; dicine, clinical decision support systems, and data-driven healthcare technologies. Despite rapid technological advancement, limited empirical evidence explains how healthcare professionals’ digital competence contributes to healthcare service performance within the context of healthcare transformation in Saudi Arabia. Previous studies have primarily focused on technological adoption or technical outcomes, while the psychological and organizational mechanisms underlying digital healthcare performance remain underexplored. Drawing upon the Job Demands–Resources (JD-R) Theory and Resource-Based View (RBV), this study investigates the influence of Digital Health Competence (DHC) on Healthcare Service Performance (HSP), examining the mediating role of Work Engagement (WE) and the moderating role of Organizational Support (OS). This study employed a quantitative cross-sectional explanatory design using Structural Equation Modeling–Partial Least Squares (SEM-PLS). Data were collected from 312 healthcare professionals at King Abdullah Medical City (KAMC), Saudi Arabia, selected through stratified random sampling. The study included physicians, nurses, pharmacists, and allied healthcare professionals actively utilizing digital healthcare systems in clinical practice. Measurement instruments were adapted from internationally validated scales, including the European Digital Competence Framework for Health Professionals, Utrecht Work Engagement Scale (UWES), and Perceived Organizational Support Scale. Data analysis included assessment of the measurement model, structural model evaluation, mediation analysis, moderation analysis, effect size (f²), predictive relevance (Q²), and model fit indices. The findings demonstrated that Digital Health Competence had a positive and significant effect on Healthcare Service Performance (β = 0.328, p < 0.001) and Work Engagement (β = 0.541, p < 0.001). Work Engagement significantly influenced Healthcare Service Performance (β = 0.462, p < 0.001) and partially mediated the relationship between Digital Health Competence and Healthcare Service Performance (β = 0.250, p < 0.001). In addition, Organizational Support significantly moderated the relationship between Digital Health Competence and Work Engagement (β = 0.217, p < 0.001). The structural model demonstrated substantial explanatory power (R² HSP = 0.683) and satisfactory predictive relevance. This study contributes theoretically by extending the application of JD-R Theory and RBV within the context of digital healthcare transformation in tertiary hospitals. The study proposes an integrated model demonstrating that digital competence functions not only as a technical capability but also as a strategic personal resource that enhances work engagement and healthcare service quality. Practically, the findings emphasize the importance of strengthening digital competency development, supportive organizational climates, and adaptive digital infrastructures to improve healthcare professionals’ performance and accelerate sustainable healthcare transformation in Saudi Arabia.

DIGITAL HEALTH COMPETENCE AND HEALTHCARE SERVICE PERFORMANCE AMONG HEALTHCARE PROFESSIONALS IN SAUDI ARABIA

Abstract: Digital transformation in healthcare services has significantly changed how healthcare professionals deliver clinical care, particularly in technology-based tertiary hospitals. Digital Health Competence (DHC) has become&#8230; a crucial factor determining service effectiveness, clinical decision quality, and operational efficiency in modern healthcare organizations. This study aims to analyze the influence of digital health competence on healthcare service performance at King Abdullah Medical City (KAMC), Saudi Arabia, with work engagement as a mediating variable and organizational support as a moderating factor. This study employed a quantitative cross-sectional design. The sample consisted of 312 healthcare professionals at KAMC selected through stratified random sampling. Data were collected using a Likert-scale questionnaire ranging from 1 to 5 and analyzed using Structural Equation Modeling–Partial Least Squares (SEM-PLS). The findings revealed that digital health competence had a positive and significant effect on healthcare service performance (β = 0.412; p < 0.001). Digital competence also significantly influenced work engagement (β = 0.528; p < 0.001), while work engagement mediated the relationship between digital competence and healthcare service performance (β = 0.216; p < 0.001). Furthermore, organizational support strengthened the relationship between digital competence and work engagement. These findings indicate that digital competence not only improves service efficiency and quality but also functions as a psychological resource that enhances healthcare professionals’ motivation and dedication. Practically, the study provides important implications for hospitals in improving healthcare professionals’ digital competence through continuous training, the development of user-friendly digital systems, and adaptive organizational support.

ANALYSIS OF TRANSFORMATIONAL LEADERSHIP AND ORGANIZATIONAL CULTURE IN PROMOTING NURSING INNOVATION THROUGH KNOWLEDGE SHARING: A QUALITATIVE STUDY IN THE MAKKAH HEALTH CLUSTER, SAUDI ARABIA

Abstract: Transformational leadership and organizational culture are key determinants of nursing innovation in complex healthcare systems. However, limited qualitative evidence exists regarding how these factors interact with knowledge-sharing&#8230; ledge-sharing practices to foster innovation, particularly within healthcare organizations undergoing transformation in Saudi Arabia. This study aims to explore how transformational leadership and organizational culture influence nursing innovation through knowledge-sharing practices in the Makkah Health Cluster, Saudi Arabia. A qualitative exploratory design was conducted within six hospitals in the Makkah Health Cluster. The study included King Abdullah Medical City, Noor Specialist Hospital, Maternity and Child Hospital, King Abdulaziz Hospital, Hiraa General Hospital, and King Faisal Hospital. A total of 48 participants, including nursing managers, ICU head nurses, and clinical nurses with at least two years of experience, were selected using purposive sampling until data saturation was reached. Data were collected between January and March 2026 through semi-structured interviews, focus group discussions (FGDs), non-participant observations, and document analysis. All interviews and FGDs were audio-recorded, transcribed verbatim, and conducted in English or Arabic based on participant preference. Data were analyzed using Braun and Clarke’s thematic analysis approach, and study rigor was ensured through triangulation, member checking, and audit trail procedures following COREQ guidelines. Four key themes emerged: (1) transformational leadership as a catalyst for psychological safety and innovation engagement, (2) organizational culture characterized by a dual structure of collaboration and hierarchy, (3) knowledge sharing as a mechanism for collective clinical learning and problem-solving, and (4) nursing innovation as an emergent, practice-based, and collaborative process. Findings indicate that transformational leadership enhances nurses’ motivation and openness to innovation, while organizational culture shapes the environment for collaboration. Knowledge sharing functions as a critical process that transforms clinical experience into practical innovation in nursing care. Nursing innovation in the Makkah Health Cluster is a socially constructed and dynamic process shaped by the interaction of transformational leadership, organizational culture, and knowledge-sharing practices. Strengthening leadership capacity, fostering collaborative organizational culture, and developing structured knowledge management systems are essential to sustain nursing innovation and improve healthcare service quality.

THE RELATIONSHIP BETWEEN EMOTIONAL INTELLIGENCE AND NURSING WORK EFFICIENCY AT KING FAHAD SPECIALIST HOSPITAL – QASSIM CLUSTER 2026

Abstract: Improving the quality of modern healthcare services largely depends on the effectiveness and efficiency of nursing personnel as the frontline providers of patient care. The complex and high-pressure work environment of specialized&#8230; pecialized hospitals requires nurses to cope with emotional demands, heavy workloads, and rapid decision-making in critical situations. These conditions make emotional intelligence one of the important psychological factors influencing nursing performance quality. This study aimed to analyze the relationship between emotional intelligence and nursing work efficiency at King Fahad Specialist Hospital – Qassim Cluster, Saudi Arabia, in 2026. This study employed a quantitative approach with a cross-sectional design. The study population consisted of all active nurses working at King Fahad Specialist Hospital, Qassim Health Cluster. A total of 312 nurses were selected using stratified random sampling. Data were collected using the Emotional Intelligence Scale and Nursing Work Efficiency Scale, both of which had been tested for validity and reliability. Data analysis was conducted using Structural Equation Modeling–Partial Least Squares (SEM-PLS) with SmartPLS 4 software. The results revealed that emotional intelligence had a positive and significant effect on nursing work efficiency, with a path coefficient of 0.642, t-statistics of 9.871, and p-value < 0.001. The R-square value of 0.58 indicated that emotional intelligence explained 58% of the variance in nursing work efficiency. Emotional regulation and empathy emerged as the dominant indicators contributing to improved communication quality, clinical decision-making, and patient care effectiveness. This study concludes that emotional intelligence is an important psychological resource in improving nursing work efficiency in specialized hospitals. The findings strengthen the Job Demands–Resources (JD-R) Theory and Emotional Intelligence Theory in explaining the relationship between psychological resources and nursing performance. This study is expected to provide theoretical contributions to the development of nursing management literature and practical contributions for hospitals in designing healthcare human resource development strategies based on psychological well-being.

ANALYSIS OF QUALITY CONTROL FOR POTASSIUM TESTING AT KASIH IBU HOSPITAL LABORATORY IN SURAKARTA

Abstract: Laboratory testing plays a crucial role in supporting patient diagnosis and monitoring; therefore, accurate and precise results are essential through the implementation of quality control (QC). Potassium level testing is&#8230; one of the key parameters requiring robust quality control because it is closely linked to patients’ clinical conditions. This study aims to analyze the implementation of QC in potassium level testing using the EasyLyte device at the Kasih Ibu Hospital Laboratory in Surakarta. The research method used was quantitative descriptive with an observational approach using secondary data from QC results for the period from September to November 2025. Analysis was performed by calculating the mean, standard deviation (SD), coefficient of variation (CV), and evaluating the Westgard rules. The results showed that the CV values for normal and abnormal controls were within acceptable limits (≤5%), and no violations of any Westgard rules were found. This indicates that the test results have good precision and there are no deviations, either random or systematic. Overall, the analytical system is in control and the instrument performance is stable. Thus, potassium testing using the EasyLyte instrument meets quality requirements and is suitable for use in laboratory services, while still requiring QC monitoring as well as routine maintenance and calibration.

THE INFLUENCE OF TALENT MANAGEMENT, CONTINUOUS IMPROVEMENT, AND PERFORMANCE APPRAISAL ON PRODUCTIVITY OF VETERINARY STAFF IN LARAS SATWA CLINIC

Abstract: This systematic literature review examined 40 high-quality empirical studies to synthesize evidence regarding how talent management, continuous improvement, and performance appraisal influence employee productivity across&#8230; s organizational contexts. The findings reveal that talent management creates direct and moderated effects on employee productivity through organizational culture and employee involvement mechanisms. Continuous improvement methodologies, including Lean Six Sigma approaches, yield simultaneous improvements in both productivity and quality outcomes, with demonstrated effectiveness in service-oriented sectors. Performance appraisal systems enhance employee productivity through clarified performance expectations, developmental feedback, and strengthened accountability mechanisms. A critical finding is that integrating these three human resource management domains produces synergistic effects that exceed the sum of isolated interventions, demonstrating multiplicative rather than additive productivity gains through mutually reinforcing mechanisms. The 40 included studies employed balanced methodological approaches including quantitative surveys, structural equation modeling, qualitative investigations, and empirical case studies, with 65% published in 2024-2025 reflecting contemporary human resource management practices. This systematic evidence synthesis establishes that veterinary clinics implementing comprehensive integrated approaches addressing talent management, continuous improvement, and performance appraisal simultaneously would achieve superior productivity outcomes while addressing the profession's unique emotional labor demands and animal welfare responsibilities.

Abstract: The emergence of the COVID-19 pandemic in 2019 has had an impact on all aspects of life, particularly global health. Indonesia is one of the affected countries. The Indonesian government has responded to the conditions of&#8230; f this pandemic, one of which is by imposing access restrictions to prevent the pandemic from spreading. This policy indirectly affects the community's activities, including the utilization of existing healthcare services. Meanwhile, available data show that one of the major issues that the Indonesian state is still dealing with is its status as the third country with the greatest number of TB (Tuberculosis) cases in the world, with some of the TB cases being classified as Drug-Resistant Tuberculosis (DR-TB).

CORRELATION BETWEEN NEUTROPHIL LYMPHOCYTE RATIO AND LIPID PROFILE IN ADULT PATIENT WITH OBESITY IN PRODIA TANGERANG CLINICAL LABORATORY

Abstract: Obesity is a condition of abnormal or excessive fat accumulation which can be a risk factor for dyslipidemia. Uncontrolled dyslipidemia can directly cause endothelial dysfunction by increasing the production of free radicals,&#8230; cals, causing Low Density Lipoprotein (LDL) oxidation and stimulating the release of inflammatory factors that can cause inflammation. Neutrophils and macrophages are the first leucocytes to appear in the early response to inflammation, so the neutrophil-lymphocyte ratio can be useful for evaluating the prognosis of inflammation as an early detector and as a marker of inflammation that can be detected simply and efficiently.  This study aims to determine the correlation between Neutrophil-Lymphocyte Ratio (NLR) and Lipid Profile in adult patients with obesity in Prodia Tangerang Clinical laboratory. This study is an analytical study with a cross-sectional approach and used purposive sampling technique. The research sample data was taken from secondary data from patient medical records. The p-value obtained using Rank Spearman correlation from the comparation of neutrophil-lymphocyte ratio with each lipid profile value is total cholesterol (p=0.003), LDL cholesterol (p=0.013), High Density Lipoprotein (HDL) cholesterol (p=0.375) and tryglicerides (p=0.316). This study found a relationship between the neutrophil-lymphocyte ratio with total cholesterol dan LDL cholesterol, but there was no relationship between the neutrophil-lymphocyte ratio with HDL cholesterol and tryglicerides in obese adult patient in Prodia Tangerang Clinical Laboratory.

EFFECT OF DELAY TIME IN URINE EXAMINATION ON ALCOHOL CONCENTRATION

Abstract: Urine alcohol testing plays a vital role in detecting alcohol consumption for clinical and forensic purposes, yet its results may be influenced by pre-analytical factors such as the delay between sample collection and laboratory&#8230; boratory analysis. This study aimed to evaluate the effect of delay time before urine examination on alcohol concentration. An experimental One-Shot Case Study design was conducted involving 44 participants in Denpasar who had consumed alcohol. Alcohol concentration was measured immediately, after a 2-hour delay, and after a 4-hour delay using the Alcohol Urine Rapid Test Dipstick Allchek, and the data were analyzed using the Kruskal-Wallis test. The findings showed that average urine alcohol levels increased numerically after a 2- and 4-hour delay compared to immediate testing; however, this change was not statistically significant (p=0.131). These results confirm that delaying urine alcohol examination up to 4 hours does not significantly alter detectable alcohol concentrations, although prompt testing is still recommended to ensure result accuracy and minimize the risk of false positives. This study provides important insights for the development of standard operating procedures regarding urine alcohol analysis in laboratory practice.

CORRELATION BETWEEN HEMOGLOBIN A1c (HbA1c) LEVELS AND MICROALBUMINURIA IN DIABETES MELLITUS PATIENTS AT PRODIA SUNTER

Abstract: Diabetes Mellitus (DM) is a disease caused by pancreatic failure in performing its function of producing insulin. A lack of insulin will lead to an increase in blood glucose levels, also known as hyperglycemia. Hemoglobin&#8230; n A1c and microalbuminuria are screening examinations used to detect the early onset of diabetic nephropathy. Diabetic nephropathy is defined as a clinical syndrome in diabetes mellitus patients characterized by persistent albuminuria, an irreversible decrease in glomerular filtration rate, and increased arterial blood pressure. The purpose of this study is to determine the relationship between Hemoglobin A1c and microalbuminuria in diabetes mellitus patients at Prodia Sunter. The research method used is quantitative analytic with a cross-sectional approach. The data collected were secondary data from the period of January–July 2024 with a total of 60 samples. Sampling was conducted using purposive sampling and met the inclusion criteria, namely patients over 60 years of age, Hemoglobin A1c value ≥7, diagnosed with diabetes mellitus, no history of thalassemia, and who had undergone Hemoglobin A1c and microalbuminuria examinations. The results of this study show a significant unidirectional positive relationship with a very strong correlation between Hemoglobin A1c levels and microalbuminuria in diabetes mellitus patients, with sig (2-tailed) = 0.000 and correlation coefficient value = 0.892. The relationship between Hemoglobin A1c levels and microalbuminuria is unidirectional positive, meaning that the higher the Hemoglobin A1c levels, the higher the microalbuminuria levels.
